Thank you for your interest in Shine Bright Academy! To ensure we can provide personalized support and prayers for every child receiving a Shine Bright Treasure Box, we kindly ask that you complete the following registration form.

1. Order Contact Information:

    • Name: Please provide your full name.
    • Address: Enter the complete mailing address where the Shine Bright Treasure Box will be sent.
    • Phone Number: Provide a phone number where we can contact you, if needed.
    • Email Address: Enter your email address so we can send updates and email confirmations about the Shine Bright Treasure Box.

2. Registration Type:

Please select one:

      • Individual Registration (1 child)
      • Family Registration (1+ child)

3. Children’s Information:

    • For each child receiving a Shine Bright Treasure Box, please provide: Child’s Name

Parent/Guardian

Your first Shine Bright Treasure Box order will include a Bible Buddy Bear, biblical teachings, cognitive tools, and supplies.
The Shine Bright Box is priced at $60.

 Re-ordering

After ordering your first box, additional monthly boxes will be priced at $35. The Shine Bright Treasure boxes will include biblical teachings, cognitive tools, and supplies.
~Bible Buddy Bear is only included in the first box.
